Can I be referred to your practice for an injection only?
Yes. Your physician must send a prescription stating exactly what injection they want done, your insurance information and any pre-authorizations/pre-certifications that are needed. The procedure will be done in our office under x-ray guidance. When completed, your follow up appointment should be made with the referring physician.
What Can I Expect from my first appointment?
Your consultation with the doctor is in a 1 hour time slot. They will discuss your history and present pain issues. This will be followed by a physical exam and brief discussion around a treatment plan for you.
Prior to your appointment you should complete the New Patient Questionnaire found in the Forms section of our website. If for any reason you are unable to complete the form online, we will be glad to mail you a copy. This questionnaire must be completed fully prior to your arrival

Things you should bring to your first appointment:
Please arrive 30 minutes prior to your appointment so we can prepare your chart.
- Insurance card
- Picture ID
- Co/pay co/insurance
- Films (i.e. MRI, XRAYS) The actual film, not just the report
- You should have your RECENT X-RAYS AND /OR MRI’S FOR THE DOCTOR TO VIEW.
- All workers compensation and no-fault patients should have ALL their case and policy numbers and the name and phone number of a contact person. You must also bring in your private insurance information to be placed on file and a referral if applicable.
- Please arrive on time. For confirmation, we will call you 24-48 hrs. prior to your appt. If you are not available, but have an answering machine, we will leave a message. If we cannot contact you for any reason, i.e. your phone is disconnected; we will cancel your appt. and fill that time with another patient.

Can I get a refill of a pain medication without an appointment?
Because many medications are controlled, you need to be seen on a regular basis to continue getting these medications. We are unable to refill or prescribe controlled substances after hours, on weekends, or on holidays. Also, we are unable to give prescription refills at surgical facilities on dates of procedure.
